Pathophysiology and epidemiology of Peripheral Arterial Disease
PAD is a common disease of the vasculature estimated to affect 12mm Americans, with the prevalence of CLI estimated to be approximately 2mm and growing as the population ages with more comorbidities. With the diagnosis of CLI, a patient suffers from poor perfusion of their legs and feet. It can manifest as intense leg pain while at rest, a wound that will not heal, or a gangrenous limb. According to the American Heart Association and American College of Cardiologists, evidence of hypoperfusion is a Class I indication for prompt surgical revascularization
Morbidity and Mortality
The Society for Vascular Surgery estimates that within a year of being diagnosed with CLI, 25% of patients will be dead, 30% will be alive but missing a limb due to amputation, 20% will be living with chronic CLI, while a mere 25% have resolution of their disease.


The burden of disease
Vascular surgery is the dark cloud of the hospital with the greatest number of 30-day readmissions of any surgical specialty. The American College of Surgeons National Surgical Quality Improvement Program (ACS NSQIP) found that the combined rate of readmission following endovascular and surgical revascularization of the lower extremities is 16% within the first month. Occlusion is one of the most common complications driving readmissions. Complications due to occlusion are expensive, as they lead to additional surgical procedures and prolonged hospital stays. One study estimated the cost of amputation at $18,600, endovascular revascularization at $17,100, and surgical revascularization at $40,100.
